  Many countries are investing in primary health care (PHC) reform, with particular attention being paid to establishing local or regional organisational structures; implementing new funding arrangements and changing the PHC workforce skills mix. This review examines what is known about the implementation and effectiveness of the different system-wide models being developed in Australia, United Kingdom and New Zealand to achieve PHC reform.

  Rural health policies over the past decade have been driven by the need to reduce health inequalities between metropolitan and rural Australia. These policies have concentrated on addressing workforce issues, targeting the medical workforce in particular. Little policy attention has focused specifically on the systematic development of sustainable comprehensive Primary Health Care (PHC) service models appropriate to rural and remote Australia. There is a need to know what model works best where, and why.

• EMERGENT THEMES IN THE SUSTAINABILITY OF PRIMARY HEALTH CARE INNOVATION
  
  

  A synthesis of the findings of five studies of sustainability of primary health care innovation across six domains (political, institutional, financial, economic, client and workforce) yielded three main themes. These were:

  •  the importance of social relationships, networks and champions;
  •  the effect of political, financial and societal forces; and
  •  the motivation and capacity of agents within the system.
